Master COMPANY POLICIES AND PET RELEASE FORM The AUSSIE PET MOBILE philosophy is to offer a stress-free grooming experience for your loved pet. At Aussie Pet Mobile we want your pet’s grooming experience to be pleasant and as enjoyable as possible. It is important to understand that some pets respond to grooming differently, even with best efforts. We make every effort to make it a safe and positive experience. Sometimes pre-existing conditions arise unforeseen to our groomers, such as skin allergies, shampoo allergies, moles, clipper sensitivity, matting and tangles, fleas, fear, behavioral issues, bone or joint sensitivity, heart condition, seizures, etc. It is helpful that you, as the pet owner, inform us of anything that you are aware of prior to grooming. MEDICAL / VACCINATIONS: You, the pet owner will advise us of any medical, physical, emotional issues, allergies, sensitivities or pre-existing conditions. These may include prior surgeries, hip or joint issues, warts, moles, ear infections or skin problems. Puppies and Kittens must have completed all their shots before the first groom. By signing this contract, owners attest their pets are current on Rabies Vaccinations. State Law and Aussie Pet Mobile require each pet to have current rabies vaccination prior to being groomed. While AUSSIE PET MOBILE adheres to a stringent sanitation procedure for all vans and equipment, we also strongly advise that each pet be vaccinated against distemper, hepatitis, parainfluenza adenovirus type 2, parvovirus, corona, leptospirosis, and bordetella prior to being groomed. Should your dog or cat exhibit any signs of being over stressed, we will stop the groom. You will be called out to the van to discuss what we are observing, and with you, determine if we should continue. Your pet’s well-being is our first priority. ACCIDENTS There is always the possibility an accident could occur. Grooming equipment is sharp, even though we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. In the case an injury does occur to your pet during the groom, the groomer will take care of the injury according to our safety and accident policy. And in the case of a serious injury he/she will inform immediately. BEHAVIORAL CHALLENGES: Master has the right to refuse grooming any animal that may be a threat to itself or any person. If we are not able to take a cat out of its carrier or get a muzzle on a dog that may bite, then we may have to refuse it for its own protection and ours. Owners MUST inform us if your pet(s) bites, has bitten, or is aggressive to people, other pets or specific grooming procedures. Muzzles may be used if necessary. Muzzling will not harm your pet and protects both the pet and the groomer. We reserve the right to refuse/stop services for such pet(s) at any time before or during the grooming process. If we arrive at your property and your pet cannot be groomed for any reason a minimum $50.00 groom fee will apply. The animal owner will be held responsible for any bites that may require medical treatment and for any property damage caused by their pet. All bites will be reported to the local authorities as required by law. SHAVING: - AUSSIE PET MOBILE Authorization for Shaving a Pet and or Flea & Tick Treatment Shaving a pet often exposes pre-existing skin conditions. If an animal is badly matted, you may find sores and tender skin under those mats. Many times the skin is so sore that the clippers may cut the skin, especially while taking mats off of a cat that has very thin skin. There is no way to avoid this, however slow or careful we are. We also use natural solutions and procedures to treat your pet for fleas and ticks. Some animals are sensitive to these solutions and procedures and may experience an allergic or other reaction that could be serious. If we notice any problems with the animal, you will be notified immediately. Your consent is required to perform this service. FLEAS & TICKS In order to prevent the transfer of fleas from one appointment to the next, it is our policy to reschedule appointments immediately following a pet with fleas, to flea bomb the van and tools, and for the groomer to change uniforms. If you suspect your pet has fleas or ticks, please inform us prior to the service. Please note that parasites are a health hazard to your pet as well as to humans. If fleas and ticks are found during the grooming process, your pet will be given a flea bath and you will be charged an additional fee. APPOINTMENTS: We will always arrive as close to the scheduled appointment time as possible, however, sometimes situations occur that will disrupt the daily grooming schedule. You will be called and advised of an arrival time when schedules change beyond our control. PRICING: Pricing varies according to each pet and is determined by breed, size and the condition of your pet. Prior to your appointment, we will confirm the treatment and the price with you. Additional charges apply for other services provided which may include but not be limited to: de-matting, de-shedding, clipping, excessive coat and flea treatments due to the additional time and materials it takes to safely complete such services. MATTING: Unmaintained coats cause a variety of health problems for your pet. Fleas, ticks, and other parasites may be lurking in the coat causing further skin infections. The skin underneath can be raw and inflamed. Severely matted coats will require a shave down. We cannot brush out a coat that is already severely matted due to the stress and the pain it will cause your pet. Your pet may be nicked or cut because we have to work so closely to the skin to remove the coat. We are careful, but it is possible an injury may occur. Shaving a severely matted coat is a very delicate process and an additional fee will be charged. For double coated breeds it is very important to brush out the undercoat. If these breeds are not regularly shedding their coats it is likely being packed against the skin causing similar skin issues to that of matted hair. Additionally, it may not be possible to complete the bathing process without addressing the packed in undercoat because soap can be trapped and not fully rinsed out. Coats in this condition will require additional blowout/brushout fees in order to properly complete the basic bath. If your pet cannot be humanly de-matted, we may ask permission to clip the hair short. We would always ask for your permission first. Aussie Pet Mobile is not responsible for any pre-existing conditions due to a matted coat. There will be an additional fee that ranges anywhere between $60-$80 dollars depending on how severe the matting is. If you would like to reschedule your grooming appointment and our groomer is outside your residence a $50.00 fee will apply. If the matting is very severe, we will request you bring your pet to your vet. CANCELLATION / NO SHOW: We require 48 hours notice to change or cancel an appointment. We will contact you by phone, voicemail, text or email to remind you of your appointment. Failure to call 48 hours in advance may result in a charge of $100.00 that must be paid prior to your next grooming appointment. Giving advance notice gives us the opportunity to fill your grooming appointment and enables us to rearrange our schedule. If we show up for an appointment and there is no one home to hand over the pet, the full price of the groom will be charged as a late cancellation fee. Customers who consistently cancel or reschedule less than 48 hours in advance may be required to prepay for their appointment at the time of booking and forfeit the payment if they cancel or reschedule less than 48 hours in advance. Aussie Pet Mobile understands that emergencies can happen and we will take those into account. Please be advised that we do refer unpaid Cancellation Fees to a local collection agency!!! RELEASE I am the owner/caregiver of this/these pet(s), and I have read and understood the foregoing cautions, printed above. I hereby release Aussie Pet Mobile from any responsibility of accidents, parasites, medical problems or other problems that may be uncovered while the grooming service is being performed. I hold harmless Aussie Pet Mobile from any damage, loss, or claim arising from any condition of the undersigned pet, either known or unknown. It is also further understood and agreed the terms of this agreement can change at any time, without notice, and will overwrite any and all prior signed contracts or releases. It is further understood this cause applies to any and all pets groomed. Therefore, I consent to and authorize the grooming of my pet(s) and I release AUSSIE PET MOBILE and its employees, franchisees or other representatives, from any responsibility or liability arising out of the performance of those services. Should my pet need veterinarian care during or after the process, I agree to pay any and all veterinarian fees for my pet’s care. We may ask to see your proof of vaccination in order to provide services. Payment for groom services is due at the time of service. I authorize Aussie Pet Mobile to send me emails and text messages. I have read and accept this release for the groom today and for all future grooming appointments. If you would like a copy of this release for your records, please ask. Tips are always appreciated but NEVER expected.
